Siltuximab is basically used in the treatment of multicentric Castleman"s disease in patients who are human immunodeficiency virus and human herpesvirus-8 negative. The efficacy of this medication on the human immunodeficiency virus positive and herpes virus positive patients showed a negative result because it didn"t bind to virally produced IL-6 in a nonclinical test study. Siltuximab works by binding itself to interleukin-6 thus preventing binding to soluble and membrane bound interleukin-6 receptors. Siltuximab interferes with the IL-6 mediated growth of B-lymphocytes and plasma cells. Therefore Siltuximab is indicated for:
- Prostate cancer
- Castleman"s disease
- Metastatic renal cell cancer
- Ovarian cancer
Prior to any Siltuximab dosage prescription, a hematology lab test has to be performed first. The medication is not supposed to be administered unto patients with severe infections not until they resolve. A discontinuation of the medication is recommended in the case of severe infusion related reactions, cytokine release syndrome and or severe allergic reactions. Live vaccines are not supposed to be administered to patients receiving Siltuximab medication because IL-6 inhibition interferes with the normal immune response unto new antigens.
