  • Less than 6 months: 3 mg/kg per day orally in 2 divided doses
  • Less than 2 years: 1 to 2 mg/kg per day orally daily as a single dose or in 2 divided doses
  • 2 to 12 years: 1 to 2 mg/kg per day orally daily as a single dose or in 2 divided doses
