Drug Name: Rituximab
Brand Name(s): Rituxan
Drug Class: Monoclonal Antibody
Treats: Leukaemia, Lymphoma, Rheumatoid Arthritis

Rituximab is a drug used in the treatment of cancer and rheumatoid arthritis. Specifically, it works to combat cancers relating to white blood cells such as different types of leukemia and lymphoma. Its anti-inflammatory properties also render it effective against arthritic conditions. It belongs to a class of drugs known as monoclonal antibodies, a recently introduced drug class that aims to mimic the body's defense mechanisms. The drug is intravenously administered. It carries with it the risk of contracting a potentially fatal condition called progressive multifocal leukoencephalopathy (PML). Though rare, patients need to remain vigilant about the early neurological signs of this disease.

Rituximab Uses

Rituximab, whether alone or in combination, is used to treat non-Hodgkin's lymphoma by interfering with the continuous growth and spread of cancerous cells. Rituximab is able to do this by killing certain blood cells in your immune system.

Rituximab can also be used to treat moderate to severe forms of rheumatoid arthritis by helping with joint pain and swelling. However, in most cases, Rituximab is only used to treat this condition if other medications have not worked.

Below you will find the different types of conditions that can be treated by using Rituximab:

  • Non-Hodgkin's lymphoma
  • Rheumatoid arthritis

Rituximab Dosage

Rituximab dosage is completely dependent on the individual that takes it, as it can be used for a variety of conditions. Note that the amount of Rituximab you take also depends on the age of the individual. In addition to that the amount of Rituximab, you also take depends on the strength of each injection and the number of times you will receive the injection. Note that Rituximab also depends on the type of condition you are treating.

Rheumatoid Arthritis

  • Adult Dose: Initial infusion of 50mg/hr and continue to increase by 50mg/hr every 30 minutes until the dose reaches its prescribed limit up to a maximum of 400mg/hr.
  • Succeeding infusions will begin at 100mg/hr and increase by 100mg/hr every 30 minutes until the dose reaches its prescribed limit up to a maximum of 400mg/hr.

Non-Hodgkin's Lymphoma

  • Adult Dose: Initial infusion of 50mg/hr and continue to increase by 50mg/hr every 30 minutes until the dose reaches its prescribed limit up to a maximum of 400mg/hr.
  • Succeeding infusions will begin at 100mg/hr and increase by 100mg/hr every 30 minutes until the dose reaches its prescribed limit up to a maximum of 400mg/hr.

Chronic Lymphocytic Leukaemia

  • Succeeding infusions will begin at 100mg/hr and increase by 100mg/hr every 30 minutes until the dose reaches its prescribed limit up to a maximum of 400mg/hr.

Rituximab Side Effects

Rituximab carries with it the risk of severe disabling consequences and even death. You will be monitored closely prior to the intravenous infusion and will be pre-medicated in the small chance you experience an allergic reaction. This drug is also associated with the neurologically disabling condition known as progressive multifocal leukoencephalopathy (PML). Though an incidence is rare, patients should be aware of any developing neurological symptoms such as disruptions in facial movement, speech and thinking.

The list below is by no means an exhaustive list of adverse effects, but instead documents some of the most commonly reported symptoms.Consult your doctor should you experience any of the following symptoms:

  • Cardiac Arrest
  • Blurred Vision
  • Infusion Reaction
  • Abdominal Pain
  • Confusion
  • Convulsions
  • Chest and Body Pain
  • Faintness
  • Swelling
  • Difficulty Breathing

Rituximab Interactions

Given that rituximab is a monoclonal antibody, you will be assessed whether you are taking other drugs in this class. This is because the risk of adverse effects increases, as well as the danger of contracting PML, which is caused by the presence of the JC virus that remains latent in the brain. Interactions increase the risk the JC virus is activated. Additionally, many drugs exacerbate already serious side-effects of rituximab.

The list below reflects some of the more serious interactions and these will be considered by your prescriber prior to administration. This is not, it must be said, an exhaustive list of potential interactions.
